Instructions: Use a desktop computer, rather than a mobile phone. Click “Fill a Copy” in the top right corner. Sign in with a Google account or create a new DocHub account. Then forward a copy of the confirmation email (Subject: "FINALIZED: MiMA Release Forms...") to us at support@mindfulmartialarts.org.

To order the gi/uniform

Please order the color “Premium Blue.” If “Premium Blue” is not available in your child’s size, please wait for it to become available. Thanks!

To order the undershirt

The undershirt, or rashguard, is an important part of the complete uniform. Please order the color Black. If the color Black is not available in your child’s size, please wait for it to become available. Thanks!

What to bring to class?

-**A large full water bottle (Important!!)**
-If your gi/uniform has already arrived, wear that!

-Face masks are optional 

If  your gi/uniform hasn't arrived:  
-For bottoms, drawstring shorts are preferred. Leggings or stretchy pants are okay, but shorts worn over the top of any leggings are preferred. 
-For the top, please bring a durable plain t-shirt that covers the shoulders and has a normal/high/crew neck.)

 

Absence Report Form

If your child is not going to attend class on a given day, please let us know in advance by text (808-400-4185). If you want to request a refund for an absence due to Covid quarantine, please fill out this form.
